AP/Lateral Thoracic spine positioning

Hi all, I'm currently a radiography intern in Australia.

I've always had problems getting a nice T-spine images because I tend to have issues getting the CR in the right spot, so a lot of the time I end up cutting off T1 or T12. My hospital's protocol is to do T-spine images lying down.

I know you're meant to be at level of T7, but does anyone have an tips on how to get the best centre point? Like, body landmarks and etc.? How do you position for a T-spine AP and Lateral?

This is not a big problem. If your IR (cassette) is long enough to include fromT1 to T12 you have to look for the upper and lower margines only. Then Center to the middle of the IR 0  out of 1 found this valuable. Do you?

For an AP T-spine you can centre to the middle of the sternum.
For the lateral view of the T - spine you can centre at the height of the inferior angle of the scapula at the axillary line. That´s a little bit lower than the AP view, but on a lateral view of the t-spine the most upper vertebrae aren´t displayed quite well (superpositioning of the shoulders).

In thoracic spine x-ray, you can increase the SID to 150 cm that might help not to cut any ROI 1  out of 1 found this valuable. Do you?

@sunsetflare, just try to familiarise the anatomical parts of your T-spine, you must know where is the T1-T12, and using a 14x17 cassette is too much enoug to cover the T-spine. 0  out of 2 found this valuable. Do you?
