One of my instructors always said to put your marker down by the bottom of your image because "there ain't nothin' there but shoulder."
Following this advice I have never placed a marker where it interfered with the anatomy of interest.

Fair enough, but there are a lot of places on an image that won't be affected by a marker.
I teach students to get a marker on the correct side and not worry about the aesthetics until they have got the basics down pat.
