I suspect this refers to the X-ray beam angulation towards the elbow. This angulation makes sense if you consider the position of the scaphoid on the lateral wrist image (google it). The angulation of the beam will potentially image the scaphoid en face without distortion

This is an old thread, but I'm new to the site and hope that nobody tried beam angulation as is stated in the previous comments! LOL To image the scaphoid, lay the forearm and hand (palm down) against the table. You would then want to angle the wrist 25-40 degrees towards the side of the 5th metacarpal (medially in the anatomic position). Hope this helps!
