What is the Value of the Lateral Chest Projection?

PA Erect Projection

This 84 year old lady presented to the Emergency Department with PUO (fever).
She was referred for chest radiography with clinical information of "septic screen".
There is abnormal opacity projected over the cardiac shadow and right mid-lower chest.
The appearances are consistent with diaphragmatic hernia of which the hiatus hernia is the most common type
However, the appearances suggest bowel content rather than stomach content

The large retrocardiac hernia is well demonstrated on the lateral projection image.
The hernia contains large bowel (small bowel does not contained faeces)
The appearances are consistent witha Bochdalek hernia
